New Top Ten: Plastic Surgery Statistics From 2009
The word is out. The American Society of Aesthetic Plastic Surgery just published it's annual statistics for cosmetic surgery.

Our own experience with women is similar, although Facelift is also very common. For men in our practice the procedures differ from national trends. We do more facelifts and necklifts along with eyelid surgery. We do liposuction, male breast reduction (gynecomastia treatment), but not rhinoplasty or hair transplants (but we do plenty of laser hair removal).
In the non-surgical realm, wrinkle treatment with botulinum toxin (Botox or Dysport) remains the most common procedure.

Low-Cost Procedures in the Local News
The local newspaper (The Greenville News, www.greenvilleonline.com) recently ran a McClatchy News Service article with the title, "Demand grows for low-cost cosmetic procedures." It is a well written article that refers to statistics from the American Society of Aesthetic Surgery and the American Society of Plastic Surgeons. Dr. Graham is a member of both organizations. www.PlasticSurgery.org and www.Surgery.org. It also quotes non-surgeons and dermatologists.
One major point is that patients are seeking more temporary "quick fixes" now.
We are proud that we offer our patients both injectable and/or laser solutions and definitive surgical rejuvenation. We suggest patients tell us the results the seek and the available down-time, and we will offer options that are suitable for them.

Plastic Surgery Top Ten: 2000 decade's biggest trends
2010's start gives us the perspective to look back over the decade of 2000 and decide what the most important trends were in plastic surgery. The publication Plastic Surgery News did this in their Jan/Feb issue and here is their list:
10) MEDICAL TOURISM increases and raises the concern about sacrificing quality for cost.
9) Increased OFFICE-BASED COSMETIC SURGERY -- now chosen by the majority of patients over a hospital or out-patient multi-use facility.
8) Increase in MAJOR WEIGHT LOSS patients, following gastric bypass or other bariatric surgery.
7) POLITICAL ADVOCACY for plastic surgery, including stopping the national cosmetic procedure 5% tax from Washington and various state bills.
6) Stressing HIGH STANDARDS. After high profile deaths including Kanye West's mother Donda, and the author of First Wives Club Olivia Goldsmith, the media and the public became more interested in the message of quality & safety repeated by the American Society of Plastic Surgery.

Eyelid cosmetic blepharoplasty in Greenville, South Carolina

Eyelid tuck (blepharoplasty) is the most common facial cosmetic surgery both nationally and in our practice in Greenville, Upstate South Carolina.
People notice our eyes as the most expressive part of our face. The skin is thinner there and years of expressions & possibly sun damage cause wrinkles, deeper lines, thinning of the lower rim, protruding fat bags, draping loose skin. Eyelid & Facelift Surgery: Attention turns to facial procedures
With summer almost over and fall approaching we notice a trend among our patients' interests. Many now want facial cosmetic procedures, often thinking ahead to looking their best for holidays, social and family gatherings.
Patients who have visited our office in the past and people who review our website have seen our strong emphasis on facial cosmetic surgery. Dr. Graham is well known for beautiful, natural looking facelifts, necklifts, short-scar cheeklifts, eyelid tuck (blepharoplasty), browlifts, laser wrinkle treatments, Botox & the new Dysport, lip enhancement, fillers such as Restylane, Perlane and Juvaderm, and finally natural fat grafts.

Safe, Experienced Injectables (Juvaderm,Restylane,Botox,etc.)
We want patients to have a great experience, whether they need standard facial rejuvenation surgery, some type of mini-lift, or enhancement with injectables such as Juvaderm, Restylane or Botox.
We recommend you do your homework. Ask about certification by the American Board of Plastic Surgery or the American Board of Otolaryngology (with Certificate of Added Qualifications in Facial Cosmetic Surgery) for surgical procedures. Ophthalmologists should also be experienced around the eye area.
Recently, doctors from all unrelated specialties have been attracted to cosmetic surgery (ie. Emergency Medicine, Obstetrics and Gynecology, Internal Medicine, General Surgery, etc.). When you find a "Medical Spa" or "Cosmetic Medicine" practice, inquire about the board certification and experience of the physician. If other personnel will be performing your injections (nurse[RN], nurse practitioner [NP-C], esthetician [non-medical] or technician [un-licensed]) you need to know even more.
Dr. Graham and Janice Sarrer NP-C have excellent background and specific additional injectable training.

Magic Wand Analogy (laser or injection): Check out the Witch or Wizard (plastic surgeon) using that thing! (copyright)
Wrinkle treatments, newest lasers, latest filler injections. We regularly get calls and we welcome questions about new developments. We are constantly looking at the many offerings of companies to find the best to offer our patients. Each new "Botox-alternative," "non-surgical facelift," "minilift" or "new laser wavelength" is a potential benefit, but quality studies are often lacking.
The "magic wand" analogy occurred to me recently. Potential patients need to ask the question posed by Glinda in the Wizard of Oz, "are you a good witch or a bad witch?" You don't need to know so much about the wand as the one waving it. The witch or wizard using the magic wand is the person who will determine the quality of your outcome.
Expert surgeons can obtain similarly excellent facelift results using a variety of techniques (SMASectomy, SMAS plication, MACS lift for example). Quality "injectors" using Botox at different concentrations or Juvaderm Ultra Plus versus Perlane can obtain superior results. The most important questions are questions about the doctor or nurse's skill, training and experience. You are not just buying a uniform product (25 units of magic neurotoxin). You are buying their medical experience.
So, my advice is don't worry so much about the wand, seek quality witches and wizards; you want the good ones.

Reloxin approved by FDA (Dysport in Europe) - Botox alternative
An alternative to Botox will be available later in 2009. Medicis Corporation, which markets Restylane and Perlane as well as other dermatologic products, announced recently that the FDA has approved its application to market an alternative botulinum toxin in the USA. Medicis has licensed the product known as Dysport in Europe from Ipsen Corporation there.
The new toxin Reloxin will work similarly to BotoxCosmetic, but with different dosages due to different activity of the product. Doctors will need to evaluate its effects on patients and adapt correct dosages.
Dr. Graham has been aware of this alternative neurotoxin for many years due to it's overseas use, and will evaluate it's benefits and risks for our patients when available here. We are one of the most active Botox users in the area, and will certainly offer proven benefits to our patients.

ASPS annual statistics are out. And the Top Five are...
The American Society of Plastic Surgeons (ASPS) is the large national organization of board-certified plastic surgeons. Their 2008 statistics were recently published. Reconstructive surgery and minimally invasive cosmetic procedures (laser, fillers, neurotoxin) increased compared to 2007.
As expected cosmetic surgery was down, but a relatively small 9%. Articles in The New York Times (April 6th, "When All You Have Left Is Your Pride") and the Wall Street Journal ("Keeping Up Appearances") may explain why patients continue to seek care to enhance their appearance. In some circles, appearance may be critical in a time of high competition for jobs. Time Magazine had a recent article about the popularity of Botox for men.
The top 5 cosmetic surgery procedures in 2008: Breast Augmentation, Rhinoplasty, Liposuction, Eyelid Surgery, and Tummy Tuck. Botox was the most popular non-invasive treatment (5 million in 2008).

"Super-charged Anti-agers," is the title of an article featuring Dr. Graham and three other area physicians, in the Beauty & Style section. Dr. Graham represented the specialty of plastic surgery. In addition there were two dermatologists and a family practitioner.

Interest in Cosmetic Procedures in a Down Economy
In addition to general reporting on the US economy, many national media reports mention the impact on cosmetic surgery and cosmetic medical procedures.
Greenville and Upstate South Carolina appear to have several advantages. Our regional economy is stronger because of our exporting sector. We continue to have employers expanding and investing. We have not seen the dramatic home price inflation of other areas. Our region remains attractive to families moving from many parts of the country.
In our practice, as summer winds down, Skin Care clients return to continue their routines and minimize the effects of past sun exposure. Minimally invasive procedures (BotoxCosmetic, Restylane, Juvaderm, MicroLaserPeels, laser hair removal, etc.) continue being popular. Breast and facial cosmetic surgery procedures continue to be the most sought after, followed by body contouring (tummy tuck and liposuction). We expect to be busier as fall season approaches.

Botox, Browlift, Blepharoplasty: How Do I Choose?
How do I tell which procedure or combination will give me an improved appearance around my eyes? Well, during an in-person consultation, we would demonstrate on your face and help you decide, but here are some hints:
1. Frown lines and squint lines or crow's feet respond to Botox treatments. A browlift may give longer lasting improvement in frown lines, but more motion will usually remain. Only the Botox will reliably improve the squint lines.
2. Excessive active lines crossing the forehead do not go away with typical browlift. Instead the entire forehead unit moves up a little (gravity has pulled it down). Botox can diminish the lines, or make them go away if they're mild.
3. Be careful about using Botox for too much forehead treatment. You may have either no natural forehead expressions (people wonder about you) or you may have drooping brows (we use our forehead muscles to lift our brows).
4. If your eyebrows are slightly too low, a little Botox beneath the inner and outer corners can give a little lift, without surgery.
5. If your eyebrows and thick skin just under the brows closes-in around your eyes, that responds best to surgical browlift. In addition, that procedure may improve your upper eyelid appearance.
6. Hooding of the eyelid skin over the lid or lashes improves with blepharoplasty (eyelid lift, tuck)
7. Puffy, thick upper eyelid folds respond to blepharoplasty.
8. Finally, a laser peel can greatly improve the texture of the eyelid skin. Sometimes that would be enough, without skin removal.
Obviously, some or all of these techniques may work together to enhance your appearance.

"Water Bottle Lips" Instead of "Smokers Lines"
Friends tell me that the national news ran an amusing item. It seems someone has decided that sucking from the spout of sports water bottles on a regular basis may cause wrinkles in the skin around the lips. Hydration is good for the skin, so don't stop drinking plenty of water.
Smoking was a problem in two ways. It seemed to accelerate skin aging chemically and smokers pursed their lips while sucking in the smoke.
