Health Reform: List prices on everything. New websites may help this happen

My view on health reform is very different from current media and congressional measures. I would like to see financially empowered individuals drive the system, rather than ever expanding government bureaucracies and insurance and pharmaceutical industries. Published prices would help individuals. All patients are shocked and amazed when they see their hospital, physician and laboratory charges (not the amounts actually paid, just what they charge before insurance contractual discounts). I would like to see all list prices widely available, in advance. (Our fees are easily available to callers, and given in writing during a consultation.)

A recent Wall Street Journal article was titled, "Lifting the Veil on Pricing for Health Care." (Wed, Oct. 28, 2009) This shows some early attempts to use the web to let people know and compare prices. It is not the same as each company giving us their prices in advance, or publishing them for all to see. That would be better, but these websites are an attempt.

The government programs have set fees for Medicare (federal, elderly & disabled)(www.cms.hhs.gov) and Medicaid (state/federal, low income). Their websites may show their allowed amounts. Most hospitals and physicians cannot remain in business on the low amounts these government programs provide, without other sources of income. They certainly cannot provide the care that many of us are accustomed to today.

Even patients with "good" insurance would benefit from the influence that comparison has in a marketplace. I think we call that transparency.

Breast lift in South Carolina by Dr. Sutton Graham


In South Carolina, a very common patient request is breast lift. Women consult Dr. Graham about changes they have gone through. Their breasts have changed by their 30's to 40's, due to having children, nursing children, loosing weight, or simply time & gravity. They notice how good they look in supportive bras, but not without good support. They notice how sports bras and tight, fitted clothing flattens the breast. We often hear, "I'm only 30 and my breasts look like my grandmother's." (My apologies to beautiful grandmothers everywhere.)

Mastopexy means to lift or tack up the breast. It is the operation we perform to improve breast sagging. There are a variety of types based on the issues that patients are experiencing. Mastopexy does not make breasts larger or smaller because it preserves your natural breast tissue. Only skin is removed.

If we simply remove skin above a low areola and nipple, the operation is called peri-areolar mastopexy. If we remove some of the loose skin beneath the breast, the operation is called a vertical-pattern, key-hole, or lollipop mastopexy. If we remove more skin from the breast, the operation is called a standard or Wise-pattern mastopexy. Incisions will fade over time and can be easily covered under swim suits, bra tops, plunging necklines, etc.

In addition to the skin removal, important internal tightening is done. The breast gland tissue and normal fat is often suspended or supported internally to improve breast projection and upper pole fullness. Many patients request the addition of a breast implant to increase breast size and enhance projection and fullness. Some patients request a size decrease, removal of a portion of their breast tissue. This is called a breast reduction. When a patient has two different sized breasts, different procedures improve symmetry.

Breast + Tummy (times 30s - 40s) = Mommy Make-0ver: That's the formula

Families cherish their children. Their birth brings us innumerable joys. Pregnancy also effects women's bodies in many ways. Sometimes there are minimal visible after-effects, however women with more pronounced changes consult Dr. Graham to improve their situation.

Breast size, shape and firmness can change. For some women, breasts become much larger and uncomfortable; breast reduction is in order. For others, breasts sag, become much smaller, and loose their firm texture. In these situations clothes don't fit well. It becomes harder to find dresses, summer tops, and bathing suits that flatter the figure. Breast lift (mastopexy) or breast augmentation (implants) can help these situations. The two may be combined, known as augmentation mastopexy. We are impressed by how trim, toned and fit some celebrities look after bearing children, but we know they have the best help and perhaps PhotoShop enhancement. Normal women's abdomens become stretched by pregnancies, some with prominent stretch marks (known as striae). Metabolism changes and body fat accumulates in different places such as the central / lower tummy and the "love handles" or flanks and iliac regions.

Cosmetic Surgery Safety: California requires pre-op medical clearance ("Donda West Law")


Dr. Graham has emphasized safety and patient comfort for almost 20 years performing cosmetic surgery. Our patients receive thorough medical history evaluation and perioperative teaching, appropriate physical examination, and evaluation of our physician anesthesiologist prior to surgery. We contact our patient's primary care doctors, and occasionally cardiologists, to request relevant health information prior to surgery. This is nothing new.


Following an unfortunate well publicized tragedy in California, a local legislator held a public contest to request suggestions for laws. As a result her law requires a preoperative medical evaluation by a physician before cosmetic surgery.


Excerpt: "Governor Arnold Schwarzenegger signed into law on Oct. 11 legislation authored by District 62 Assemblymember Wilmer Amina Carter that would requires a health check and a written clearance before being allowed to undergo plastic surgery. Assembly Bill 1116, named the Donda West Law, was inspired by the death of Kanye West's mother. The bill came as a result of her 62nd Districtwide contest, "It Ought to Be a Law." "This law will potentially saved lives," Assemblymember Carter said. "There is a growing trend of people opting to have elected cosmetic surgery, and they are not aware of the risks involved." Specially, the law requires physicians and surgeons, including people licensed by the state holding permits to perform elective facial cosmetic surgery to complete a physical examination including a complete medical history on their patients, prior to performing elective cosmetic surgery"

Droopy eyelid skin; Removal is most common facial cosmetic surgery

Blepharoplasty is the medical term for a procedure often called eyelid tuck or eyelift. Removal of hanging, loose, excess upper lid skin is the most common feature that patients request to freshen their appearance, make them look less tired or old. At Dr. Graham's Aesthetic Center for Breast & Cosmetic Surgery we constantly interact with women regarding breast health. A smaller portion of our practice is post-mastectomy reconstruction, and we have cared for breast cancer patients for 19 years. Mommy Make-over is such a cute term: thoughts by Sutton Graham, MD


I don't know came up with the term, but it is certainly cleaver, "mommy-make-over." For over 15 years, we have been addressing women's concerns about their post-pregnancy changes (post-partum) with a variety of solutions for the breast and body, plus measures for the facial pigmentation, etc.

Breast enlargement without implants: fat grafts / liposelection

Several recent media reports reference using fat grafts (fat transfer or liposelection) to enhance one's body contour (buttocks, breasts, cheeks, etc.). It is even showing up on Facebook. Fat grafting is not a new idea, and plastic surgeons have used it for decades. The techniques have improved; harvesting and preparing patient's own fat for injection elsewhere in their body. The fat cell material attaches to blood vessels in the recipient area, and becomes a living part of your body, similar to a skin graft healing in place of a skin burn wound. Current techniques mean that a higher proportion of the transferred fat continues living in the new location. Most surgeons plan some degree of over treatment, because they expect more than one-third of the fat will be re-absorbed. Staged treatments are sometimes planned.

In our practice, fat transfer is most common for loss of attractive facial fullness (lips and cheeks), restoring a soft natural fullness. We also use it for buttock enhancement, usually in conjunction with liposuction of excess fat from the surrounding areas. It can be used to correct irregularities from prior injury or trauma (indentation in your normal fat). Dr. Graham is interested in the results of current studies that confirm the safety and effectiveness of fat graft augmentation of the breast. If the problems are loose skin, stretch marks across the lower abdomen, a roll-like collection of fat, and / or poor tone of the abdominal wall then tummy tuck can improve your appearance. If you lost quite a bit of weight then you often experience some of these. On the other hand if the problem is only localized fat that doesn't respond to diet and exercise, then liposuction may be a better procedure. The procedure is done as an out-patient in our nationally accredited office surgery facility. You need assistance for a few days and will normally resume driving in 1 1/2 or 2 weeks. You wear a support girdle for 3 weeks post-op. We recommend starting vigorous exercise 6 weeks post-op. The recovery is more significant than for most other cosmetic surgery procedures, however we use special new techniques such as a continuous-infusion local anesthetic pump for 3-4 days after the procedure. This reduces you discomfort and decreases the need for narcotic pain medicine.

Tummy tuck is often combined with liposuction of the flanks and hip areas to improve the contour of the entire torso. Some patients choose to have breast enhancement (breast lift, breast augmentation, or breast reduction) during tummy tuck. Occasionally we do unrelated procedures such as lip augmentation by fat graft or liposuction of the neck and under-chin area.

Q: What are you board certified in? A: Plastic Surgery


Patients seeking cosmetic surgery from a plastic surgeon should realize that many other types of licensed physicians perform cosmetic procedures, even surgical procedures. In addition to reputation and recommendations from your trusted physicians, you may want to confirm that the doctor you plan to see is board certified in plastic surgery.


The American Board of Medical Specialties (http://www.abms.org/) is our nation's most respected and established organization that oversees board certification. It only approves 24 medical specialty boards. There are hundreds of additional organizations that issue "board certifications" without meeting to high standards of the ABMS. Saline implants are well known for their long-term safety. Their use was never restricted by the FDA. Subpectoral saline implants have an excellent record; low rates of capsular contracture, good compatibility with mammograms and attractive transition between the upper chest and the upper breast.

We have an approach to selection of implant size that pleases and informs. We assess your desires, your current cup size, changes with prior / future pregnancies, changes from weight gain or loss, your breast gland thickness and firmness, and finally your skin looseness. We show you a range of sizers, and you try them on in a bra here with our experienced staff. You get to see approximately how your figure will look after surgery.

Appropriate pre-medicaton, surgical anesthesia (by our board-certified MD), and post-op medications provide comfort. Most women are decreasing the pain pills in a few days. Patients resume most normal activities quickly. We only ask that you hold off on your vigorous exercise routines for a few weeks. Of course, the results are obvious right away.

Clemson Cosmetic Surgery Presentation, Tuesday October 27th

Nipples Inverted, Too Large, Areolas Too Big: We can help at Dr. Graham's Greenville practice


Many women are surprised to learn that there are easy answers for "small" problems with their nipples and areolas. Some women specifically make appointments for Dr. Graham to advise them on correction of problems they have with the appearance of their nipples. Others learn during a consultation for breast augmentation, breast lift, or breast reduction that they do not have to simply live with unusual appearing nipples. If the two are quite different, we can make them more alike.


Inverted nipples are able to be brought out. (Newly inverted nipples should first be evaluated by you primary care doctor, but long-term inversion is more common.) Elongated nipples can be made average size. These procedures are often combined with another breast surgery, but can be done by themselves. In fact, they are easily performed under local anesthesia in a brief procedure, with minimal recovery, and oral medication. The areola is the specially pigmented area around the nipple. If the areola is very wide, or elongated, some women request a smaller, more circular size. Sometimes the two are very asymmetric. By itself, the correction can be easily done under local anesthesia. If other breast concerns are present, areola correction can be combined with the breast procedure. In fairly unusual situations, a woman has abnormal breast development that effects the areola and breast tissue. In a condition called tubular breast deformity, the areola becomes dome-like, with breast tissue pushing out, and little natural breast shape. Eyelid cosmetic blepharoplasty in Greenville, South Carolina


Eyelid tuck (blepharoplasty) is the most common facial cosmetic surgery both nationally and in our practice in Greenville, Upstate South Carolina.

People notice our eyes as the most expressive part of our face. The skin is thinner there and years of expressions & possibly sun damage cause wrinkles, deeper lines, thinning of the lower rim, protruding fat bags, draping loose skin. Gravity pushing the forehead and eyebrows down adds to the tired or sad impression that the eyes can give.
