Breast Lift: help for natural or excessive sagging
Some women come to us with what they think is a relatively uncommon request, "can you put my breasts back where they were?" It is one of the most common issue we help solve.
Breast lift is a procedure the can be done in many ways, adapted to each woman's requests. Provide more projection, cleavage, upper pole fullness. Restore the nipples to the front of the breasts. Move the glandular tissue back to the upper chest (and off the lower ribs). There a variety of ways surgeons use to accomplish these goals.
As plastic surgeons, we want to achieve the best look and function with the least visible incision scars. All breast lifts leave a scar around the areola (except when a woman's goals can be accomplished by implant augmentation alone - no actual lift). When doing a small amount of tightening of the lower gland, a vertical incision is needed. This may be called the vertical mastopexy, key-hole, or lollipop pattern. When there is even more looseness of the gland or skin, we also remove an ellipse of skin beneath the breast, so that this scar is hidden along the inframammary (under-breast) fold. This scar pattern is called the inverted-T or Wise pattern. The latter is most often used when reducing the size of large, heavy breasts.

Laser Resurfacing: How to Smooth Rough Skin
Medical lasers do a wide variety of things, depending on wavelength, power delivery, etc. We have a number of lasers available to treat conditions ranging from acne, roseacea, unwanted hair, to skin looseness. Dr. Graham and our certified nurse practitioner (Janice Sasser, RN, NP-C) both offer laser treatments, from minor to extensive, under safe conditions and appropriate anesthesia.
More superficial lines will be treated with an anesthetic cream and surface cooling devices. Eye protection is always used. The recovery time is a few days. These treatments are good for mild-to-moderate lines, short recovery times, and patients considering staged treatments. Deep lines will be treated with local or general anesthesia. Recovery time is several days to two weeks. These treatments give more effects and have more recovery.

Lines beyond the corners of the mouth may benefit, if other indications exist, from a facelift. Finally old, deep acne scarring in some adults can be helped by both laser and facelift. Holiday Food Drive - Support Harvest Hope Food Bank
Throughout this month, Dr. Graham & him staff will be showing our support for our local Harvest Hope Food Bank.
Harvest Hope is the largest charitable hunger relief organization in the US. It has three offices in South Carolina (Greenville, Columbia & Florence). They receive donations from individuals, grocery stores, wholesalers and distributors and distribute to 400 direct service agencies (family shelters, food pantries, soup kitchens and after school programs). They operate an emergency food pantry serving 69,000 people each year. There is also a mobile food pantry serving eight rural SC counties without emergency food programs.

We have a collection barrel in our lobby. We are asking patients coming to our office to contribute non-perishable food items, personal and baby care items, or pet food when they visit us for consultation or follow-up visits throughout the month of December.

Breast Lift Quality in South Carolina; With or Without Implants

We have the pleasure of treating a variety of women seeking breast lift. For example, many women are healthy and active following child-bearing and breast-feeding. They look good in many of their clothes, but need effective bra support to maintain attractive breast shape. They have trouble wearing bathing suits, sundresses, plain tee-shirts, gowns and evening wear; all because they feel they need good support.
Breast lift can change that. You can look better when your outfit is braless, or look even better wearing a bra. Firm support will be unnecessary. Finally, after a breast lift, you will find that sports bras give you more effective support and comfort when jogging, playing tennis, or other vigorous sports.
Another common breast lift patient is a woman who has succeeded in loosing considerable excess weight. She may have done this by diet & exercise, or weight-loss (bariatric) surgery. While she may be pleased with her slimmer physique, she may report dramatic loss of breast shape. Mastopexy breast lift is capable of removing large amounts of loose skin, while preserving all natural breast glandular tissue.
Many breast lift patients want a lift and a larger breast size. Mastopexy is often combined with augmentation (implants). This can give an excellent youthful look to your breasts. It can restore pre-pregnancy breast size and shape. Or it may improve on what you already have.
Breast lift operations are designed to preserve your nipple and areola sensation. There may be temporary decreases, but normal sensation usually returns, with most procedures. Similarly, women usually maintain the ability to breast-feed, although breast lift is not commonly indicated before having children. Many people ask us about having cosmetic plastic surgery during the holiday season -- Thanksgiving, Christmas and New Years.
Some find it surprising that anyone would want to miss the typical gatherings, travel and celebrations that are so common around that time. Many businesses slow their pace around the holidays. Planning, decorating, gift purchases, etc. seem to take additional time. Many people take time off from work.
Patients consult Dr. Graham, often months in advance, to have surgery done and recover during the holiday period. They are often very organized, having planned this well in advance, and often completed other preparations months ahead. They use the holiday breaks to allow them time to recover while others are traveling or celebrating, and a week or so later they are back at their routines. Frequently, other people haven't noticed their absence, but can see how much better they look.

Taxes on Cosmetic Surgery Patients

PLASTIC SURGEONS RESPOND TO PROPOSED COSMETIC SURGERY TAX
SURPRISE ADDITION TO HEALTHCARE BILL IS ARBITRARY, DIFFICULT TO ADMINISTER
Arlington Heights, IL and New York, NY: – Late yesterday, democratic leaders in the Senate unveiled their proposal for overhauling the health care system, which included a new 5% tax on elective cosmetic procedures. Senate Democrats argue that the tax, which was a surprise addition to the sweeping 2,074-page bill, will generate $5.8 billion over the next 10 years to be put towards the bill’s estimated $849 billion price tag. However, the American Society of Plastic Surgeons (ASPS) and the American Society for Aesthetic Plastic Surgery (ASAPS) oppose this tax as discriminatory, arbitrary and ineffective.
“Elective surgery taxes discriminate against women, given that 86 percent of cosmetic surgery patients are female, of which 91 percent are between the working ages of 19-64,” said Dr. Michael McGuire, ASPS President. “Moreover, contrary to popular belief, cosmetic surgery is no longer an exclusive luxury afforded by the very wealthy, but rather a mainstream and reasonable option most common amongst the working middle-class.”
"This tax is effectively a “Soccer Mom” tax that will adversely impact mainstream American wives and mothers, who are the majority of plastic surgery patients," said Renato Saltz, MD, President of the American Society for Aesthetic Plastic Surgery (ASAPS). “As doctors, we understand and appreciate the need for health care reform, but taxing physicians and cosmetic surgery procedures to pay for the reform is not realistic or beneficial,"
In a 2005 ASPS survey of people planning to have cosmetic surgery within the next two years, 60% of respondents reported an annual household income of $30,000-$90,000 a year. Most importantly, 40% of those reported a household income of only $30,000-$60,000. Only 10% of respondents reported a household income of over $90,000, which clearly refutes the suggestion that elective surgery taxes are “luxury” or “sin” taxes affecting a privileged few.
The ASAPS and ASPS also contend that such taxes have been proven arbitrary and difficult to administer as evidence by the experience in New Jersey, the only state to have imposed a cosmetic tax. Since it passed a 6% tax on elective medical procedures in 2004, the NJ Department of Taxation has experienced a 59% shortfall based on projected revenue estimates. Eight other states have considered similar tax legislation– and they all wisely rejected them.
Finally, though the bill claims that the only procedures that would be taxed are those that are “not necessary to ameliorate a deformity arising from, or directly related to, a congenital abnormality, a personal injury resulting from an accident or trauma, or a disfiguring disease,” The Plastic Surgery Societies contend that such distinctions aren’t always obvious. Because the line between ‘cosmetic’ and ‘reconstructive’ surgery is not always clear, such a tax would leave the determination of medical necessity up to state tax auditors – a completely inappropriate proposition.
“As doctors our first concern always needs to be patient safety,” said Dr. Saltz, “for those patients here in this country, as well as those patients who may now be encouraged by this tax to obtain their procedures abroad.”
“The ASPS has, and continues to oppose all taxes on physicians, in any and all forms, due to their deleterious effects of health care costs and access to patient care,” said Dr. McGuire. “Medical care should not be used as a tool to fix broken finances.”
The American Society of Plastic Surgeons represents more than 7,000 physician members and is recognized as a leading authority and information source on cosmetic and reconstructive plastic surgery. ASPS comprises more than 94 percent of all board-certified plastic surgeons in the United States. "My eyes make me look so tired." Eyelid blepharoplasty can fix that.

Some times patients notice it themselves. Sometimes it's because someone asks, "Why do always look so tired? Are you getting enough sleep?"
People judge our emotions and health by how we look to them. Hanging, loose upper eyelid skin or lower eyelid bags cause others to assume we are not our best.
Eyelid tuck, eye lift, or its medical name "blepharoplasty" is the procedure that is done to correct those characteristics. Under light anesthesia, or occasionally local, extra skin is removed, fat pockets are smoothed, crinkly skin is tightened with laser, etc. These procedures may be done only to the upper or the lower lids, or both. Public education success in Upstate South Carolina
We were very pleased with the response to our public education efforts last night.
Dr. Graham, our patient coordinator Betty Babb, and our nurse practitioner Janice Sasser hosted a well attended presentation at a beautiful restaurant meeting room in Seneca, South Carolina (Rudy's on Ram Cat). They had some great appetizers and refreshments for our guests.
We presented information on cosmetic surgery, cosmetic medical procedures, and professional skin care. We covered breast & body, and facial procedures in two, back-to-back sessions. We offered Dr. Graham's presentation, interesting statistics, before & after photographs, periods for audience questions, and time for individual questions. We were pleased with the numbers of people in attendance and their interesting questions. Thanks to all of you that came and shared the evening with us. We hope you'll want to do it again sometime.

Health Reform: List prices on everything. New websites may help this happen
My view on health reform is very different from current media and congressional measures. I would like to see financially empowered individuals drive the system, rather than ever expanding government bureaucracies and insurance and pharmaceutical industries. Published prices would help individuals. All patients are shocked and amazed when they see their hospital, physician and laboratory charges (not the amounts actually paid, just what they charge before insurance contractual discounts). I would like to see all list prices widely available, in advance. (Our fees are easily available to callers, and given in writing during a consultation.)
A recent Wall Street Journal article was titled, "Lifting the Veil on Pricing for Health Care." (Wed, Oct. 28, 2009) This shows some early attempts to use the web to let people know and compare prices. It is not the same as each company giving us their prices in advance, or publishing them for all to see. That would be better, but these websites are an attempt.

They certainly cannot provide the care that many of us are accustomed to today.
Even patients with "good" insurance would benefit from the influence that comparison has in a marketplace. I think we call that transparency.

Breast lift in South Carolina by Dr. Sutton Graham

In South Carolina, a very common patient request is breast lift. Women consult Dr. Graham about changes they have gone through. Their breasts have changed by their 30's to 40's, due to having children, nursing children, loosing weight, or simply time & gravity. They notice how good they look in supportive bras, but not without good support. They notice how sports bras and tight, fitted clothing flattens the breast. We often hear, "I'm only 30 and my breasts look like my grandmother's." (My apologies to beautiful grandmothers everywhere.)

Mastopexy means to lift or tack up the breast. It is the operation we perform to improve breast sagging. There are a variety of types based on the issues that patients are experiencing. Mastopexy does not make breasts larger or smaller because it preserves your natural breast tissue. Only skin is removed.
If we simply remove skin above a low areola and nipple, the operation is called peri-areolar mastopexy. If we remove some of the loose skin beneath the breast, the operation is called a vertical-pattern, key-hole, or lollipop mastopexy. If we remove more skin from the breast, the operation is called a standard or Wise-pattern mastopexy. Incisions will fade over time and can be easily covered under swim suits, bra tops, plunging necklines, etc.
In addition to the skin removal, important internal tightening is done. The breast gland tissue and normal fat is often suspended or supported internally to improve breast projection and upper pole fullness. Many patients request the addition of a breast implant to increase breast size and enhance projection and fullness. Some patients request a size decrease, removal of a portion of their breast tissue. This is called a breast reduction. When a patient has two different sized breasts, different procedures improve symmetry.

Breast + Tummy (times 30s - 40s) = Mommy Make-0ver: That's the formula
Families cherish their children. Their birth brings us innumerable joys. Pregnancy also effects women's bodies in many ways. Sometimes there are minimal visible after-effects, however women with more pronounced changes consult Dr. Graham to improve their situation.
Breast size, shape and firmness can change. For some women, breasts become much larger and uncomfortable; breast reduction is in order. For others, breasts sag, become much smaller, and loose their firm texture. In these situations clothes don't fit well. It becomes harder to find dresses, summer tops, and bathing suits that flatter the figure. Breast lift (mastopexy) or breast augmentation (implants) can help these situations. The two may be combined, known as augmentation mastopexy. We are impressed by how trim, toned and fit some celebrities look after bearing children, but we know they have the best help and perhaps PhotoShop enhancement. Normal women's abdomens become stretched by pregnancies, some with prominent stretch marks (known as striae). Metabolism changes and body fat accumulates in different places such as the central / lower tummy and the "love handles" or flanks and iliac regions.

Cosmetic Surgery Safety: California requires pre-op medical clearance ("Donda West Law")

Dr. Graham has emphasized safety and patient comfort for almost 20 years performing cosmetic surgery. Our patients receive thorough medical history evaluation and perioperative teaching, appropriate physical examination, and evaluation of our physician anesthesiologist prior to surgery. We contact our patient's primary care doctors, and occasionally cardiologists, to request relevant health information prior to surgery. This is nothing new.
Following an unfortunate well publicized tragedy in California, a local legislator held a public contest to request suggestions for laws. As a result her law requires a preoperative medical evaluation by a physician before cosmetic surgery.
Excerpt: "Governor Arnold Schwarzenegger signed into law on Oct. 11 legislation authored by District 62 Assemblymember Wilmer Amina Carter that would requires a health check and a written clearance before being allowed to undergo plastic surgery. Assembly Bill 1116, named the Donda West Law, was inspired by the death of Kanye West's mother. The bill came as a result of her 62nd Districtwide contest, "It Ought to Be a Law." "This law will potentially saved lives," Assemblymember Carter said. "There is a growing trend of people opting to have elected cosmetic surgery, and they are not aware of the risks involved." Specially, the law requires physicians and surgeons, including people licensed by the state holding permits to perform elective facial cosmetic surgery to complete a physical examination including a complete medical history on their patients, prior to performing elective cosmetic surgery"

Mommy Make-over is such a cute term: thoughts by Sutton Graham, MD

I don't know came up with the term, but it is certainly cleaver, "mommy-make-over." For over 15 years, we have been addressing women's concerns about their post-pregnancy changes (post-partum) with a variety of solutions for the breast and body, plus measures for the facial pigmentation, etc.

Breast enlargement without implants: fat grafts / liposelection

Fat grafting is not a new idea, and plastic surgeons have used it for decades. The techniques have improved; harvesting and preparing patient's own fat for injection elsewhere in their body. Q: What are you board certified in? A: Plastic Surgery

Nipples Inverted, Too Large, Areolas Too Big: We can help at Dr. Graham's Greenville practice

Many women are surprised to learn that there are easy answers for "small" problems with their nipples and areolas. Some women specifically make appointments for Dr. Graham to advise them on correction of problems they have with the appearance of their nipples. Others learn during a consultation for breast augmentation, breast lift, or breast reduction that they do not have to simply live with unusual appearing nipples. If the two are quite different, we can make them more alike.
Inverted nipples are able to be brought out. (Newly inverted nipples should first be evaluated by you primary care doctor, but long-term inversion is more common.) Elongated nipples can be made average size. These procedures are often combined with another breast surgery, but can be done by themselves. In fact, they are easily performed under local anesthesia in a brief procedure, with minimal recovery, and oral medication. The areola is the specially pigmented area around the nipple. If the areola is very wide, or elongated, some women request a smaller, more circular size. Sometimes the two are very asymmetric. By itself, the correction can be easily done under local anesthesia. If other breast concerns are present, areola correction can be combined with the breast procedure. In fairly unusual situations, a woman has abnormal breast development that effects the areola and breast tissue. In a condition called tubular breast deformity, the areola becomes dome-like, with breast tissue pushing out, and little natural breast shape. Eyelid cosmetic blepharoplasty in Greenville, South Carolina

Eyelid tuck (blepharoplasty) is the most common facial cosmetic surgery both nationally and in our practice in Greenville, Upstate South Carolina.

Office seminar last night: cosmetic surgery education by Dr. Sutton Graham
Last night, Tuesday, we hosted an in-office patient education seminar. It attracted a wonderful group of people who were treated to refreshments, an office tour, presentation by Dr. Graham including new developments & trends, and a variety of before and after photos.
The audience included our patients who were interested in new information about procedures, and many new faces we welcomed for the first time. Male Breast Reduction in South Carolina: Gynecomastia Treatment
Although most of Dr. Sutton Graham's breast surgery patients are women, a significant number are men with excessively enlarged breasts. The photograph shows correction of a small to moderate degree of male breast enlargement; a condition known as gynecomastia. We provide correction for patients with this condition.
Men with breast enlargement experience embarrassment about their appearance, often being unwilling to participate in sports, especially swimming, or other outdoor activities where they would normally have their shirts off. It can be more noticeable at the gym. We want them to be able to enjoy a healthy and active lifestyle.
The consultation is the first step. We will evaluate the degree of the problem (excess skin, fat and fibrous or glandular tissue), check for lumps, evaluate your over-all health, and check for any rarer causes such as drug side effects, hormone diseases or tumors. The average patient experienced enlargement early in their teens and it continues into adulthood. Details of the procedure are discussed thoroughly. In most cases, surgery is the recommended treatment, and can be only liposuction of localized fat or excision of fibrous and glandular tissue through a tiny concealed incision part of the way around the areola / nipple. Occasionally there is so much loose skin that a larger incision is used to tighten skin and remove abnormal gland. The patient can return to work and most activities in several days.
During the same procedure, some men choose additional liposuction body contouring: abdomen, flanks, "spare tire," or under chin / neck areas are most common. This limits recovery and down-time and improves other potential problem areas. Liposuction is best for treatment of localized fat deposits that don't respond to diet and exercise.

Breast Lift / Mastopexy in Greenville, South Carolina, by Dr. Sutton Graham
One of the most common concerns women bring to our practice is sagging or drooping of their breasts. Often they report starting with nice breasts, but time, pregnancies and breast feeding have caused their breasts to loose their once-perky contour. Sometimes, this has been associated with loosing volume in their breasts too.Down-ward pointing nipples, flattening of the upper contour, loss of upper fullness, hanging skin, etc. We hear about not being able to find attractive bathing suits (needing more support), not being able to wear sundresses or other tops that lack support, looking saggy or older, etc.
Breast lift surgery can be a solution. You achieve more youthful contour, better projection, more upper fullness, and less breast hanging onto your upper ribcage. And the nipples can point forward again. If you have lost volume or want more fullness, a moderate sized breast implant can be added as well. (Known as mastopexy with augmentation)
The procedure is done as an out-patient, with return to work in 4-7 days for most patients. You can resume work-outs in about 3 weeks. Healing takes place over time and scars fade over several months (you can see this in the photo above).
